Before scheduling a Medicaid NEMT ride, please review our service area map and general guidelines below. After verifying your eligibility, please click the button below or at the bottom of the page to fill out a ride request online.

MedRide’s NEMT services are NOT available for: running errands, completing job interviews, or completing other paperwork.

However, we can now provide transportation to pick up your prescriptions.

Please note that submitting your ride request online does not guarantee your ride will be approved, because our team must first verify Medicaid Member Eligibility. Also, our ride request form and all Medicaid transportation information on this page are subject to change, as we receive continual legislative updates from the State of Colorado and Medicaid.

STEP 1

Check your eligibility for NEMT.

In order for a patient to be eligible for Colorado’s NEMT transportation services, they must first match all of the following criteria:

  • Be a current Colorado Medicaid patient through Health First Colorado (Colorado’s Medicaid Program)
  • Requests for transportation must be for a Health First Colorado covered service and to a covered Health First Colorado provider
  • Be determined not to have access to any other means of transportation, including free public transportation options
  • Require Non Emergency, Medicaid eligible transportation. In the case of an Emergengy, please dial 9-1-1 instead.
  • MUST NOT BE ENROLLED in any exclusionary program:
    • Cannot Be a Qualified Medicaid Beneficiary (QMB)
    • Cannot Be a Special Low Income Medicare Beneficiary (SLMB)
    • Cannot Be a Medicare Qualifying Individual-1 (Ql-1)
    • Cannot Have Emergency Medicaid (EMS) and Reproductive Health Plan Coverage
    • Cannot Have an Old Age Pension – State Only (OAP- state only)

NOTE: CHP+ is not a Medicaid program, and therefore is not an eligibility group for our NEMT services.

STEP 2

Gather your personal information.

Once you determine your eligibility, have reviewed our service area map, and before scheduling your Medicaid transportation, please have the following information ready on hand:

  • Your HealthFirst Colorado ID Number (Medicaid ID#), First & Last Name, and Date of Birth
  • The Pick-Up Address where we will be picking you up to begin your Medicaid transportation ride
  • The Doctor’s Name & Address, as well as a good Telephone Number for confirmation of your medical appointment
  • The Date & Time of your scheduled medical appointment or service
  • If your destination is over 25 miles from your pickup location, the state of Colorado also requires you to fill out a 25+ Miles Form per appointment location. If this applies to you, please click the button below to download the latest form from the state’s website, completely fill out that form, and then email it over to Applications@MedRideCo.com or send it to MedRide via Fax to: 719-545-0499

Please note the ONLINE scheduling restrictions below:

Online ride requests submitted Monday-Friday require at least 24 hours’ notice.

Online ride requests submitted on Saturday or Sunday will NOT be processed for trips occurring on a Monday. All Monday trips must be submitted online by no later than Friday at 5:00PM.

Online ride requests submitted on Saturday or Sunday for trips occurring on Tuesday-Friday require at least 48 hours’ notice.
